An ISO 17065 checklist follows the clauses: legal entity and agreements, impartiality, structure and mechanism, people and resources, the process from application to complaints, and the management system.
A useful checklist follows the standard rather than a generic ISO 9001 template. Clause 4 covers legal responsibility, the certification agreement, marks, impartiality, liability, access, confidentiality and public information. Clause 5 covers the structure, the authority for each function and the impartiality mechanism. Clause 6 covers sufficient and competent people, their contracts, and internal and outsourced evaluation resources. Clause 7 covers the process: scheme, application, application review, evaluation, review, decision, documentation, directory, surveillance, change, suspension and withdrawal, records and complaints. Clause 8 covers the management system under Option A or B. Each line needs the evidence an assessor samples, not a tick. The clause record on this site gives that evidence for every clause.
What evidence does an assessor sample?
The clause page for each requirement lists the artefacts. The common pattern is a current procedure, a live record, a named owner and a decision that shows the procedure was used.
Is the checklist a compliance score?
No. A checklist records what was found. It does not certify the body and it does not replace the assessor judgement.
Where this comes from
The clause record states each requirement in our own words and cites the clause. The revision page separates the public FDIS record from the edition in force. The official record is ISO standard 94443.
Not legal advice; does not certify. The FDIS requirement wording is not held and is not stated.
